Please use the form below to securely submit all of the required supporting documents for your SAPS Firearm Licence. Note that all of the information is required, so please do not begin uploading until you have the following:

• Proof of Address

• Copy of your ID Document (front and rear if credit card ID)

• Three written interview questionnaires - DOWNLOAD TEMPLATE

- Two interview questionnaires must be completed by friends, neighbours or colleagues.
- One interview questionnaire must be completed by your spouse.
(If not married or divorced - one interview questionnaire must be completed by any family member)

• Two written testimonials

- Obtained from any other person who provided an interview questionnaire.
- It must be two written testimonials (signed) from any other person who might have knowledge of the
applicant. (They must specify to your character and why they support your application).

• Copies of Proficiency Training Certificates.

• Copies of Statement of Results.

• Copy of your SAPS Competency Certificate.

• Details and copies of any existing firearm licences.

• Pictures of your safe.

- One image showing the open safe to display how it is bolted to the wall / floor.
- One image showing the closed / locked safe showing it's locking mechanism.
